Primary school application forms

The information and application forms on this page are for primary school services only. (For secondary school applications, please see this page.)

Please note that Education Transport staff are following Government guidance and are now home working due to Coronavirus. Regretably this means we are unable to post out paper forms by request until further notice.

Renewals for existing passengers for September 2020

If your child was registered during the 2019-2020 school year, we have automatically renewed everyone for the new school year starting September 2020 - except those who will progress to secondary school - so there are no forms to complete and nothing further for you to do.

However, if your child currently qualifies for statutory zero fare, you must still undergo renewal procedures to continue with your qualification from September and we will only re-register your child as a cash fare payer on their bus until confirmation of qualification is received. Changing your details: we have used all your existing registration details (stops, days of travel, contacts etc) and carried them over to the new school year. However, if you wish to make any changes or cancel your registration, please telephone 0113 3481122.


New applications for September 2020

To make a brand new application, please follow these instructions very carefully. You are advised to use either a desktop or laptop PC to work on the forms - success may vary with tablets and mobile telephone devices.

  • Click one of the links below for the form
  • Save the file to your computer
  • Open the file in your word processor application
  • Complete the form by typing only in the yellow shaded boxes - do not type outside of these boxes
  • Save the file in any format (preferably PDF if possible) and attach to an email
  • Send the email to educationcustomerservices@westyorks-ca.gov.uk with the subject line 'Primary school application'
  • You must send the form as an attachment to the email NOT as text pasted into the body of the email otherwise we cannot extract the data.

Full services: the following services are full: K71 NLC / P35 Sandy Lane / P67 Badsworth / P93 St Cuthberts, St Barnabas, Margaret Mcmillan and Lister schools. Any applications received will be kept on file and we will contact you if space becomes available.

WG2 Dane Royd School

To travel on this route your child must live in Woolley Grange and qualify for statutory zero fare. Two separate applications must be made - one to Wakefield Council for the zero fare status here and one to ourselves using the new starter form listed above. Wakefield Council will process the zero fare element then send an approval to us. When we are in receipt of both applications, we will issue a zero fare Boarding Card valid on the WG2.
