All the forms on this page are for the school year September 2018 - July 2019.

MyBus primary school application form

This application is for MyBus routes only for the school year September 2018 to July 2019.

This is an online form and there is nothing to download. At the end of the application, you will see a reference number on screen confirming receipt along with a summary of your submission - there is no need to call to confirm receipt. Please keep this reference as without it, we cannot locate your application should you make any further enquiry with us.

Please note, as at 28th November 2018:

* Services P67 Badsworth School, K71/K72 Netherhall Learning Campus, P40 Sacred Heart, P66 Baildon, P78 Thornto, P93 Cuthberts/Barnabas/Lister/Margaret McMillan, S20 Lady Elizabeth Hastings, P76 Holy Rosary & St. Annes are FULL. Applications will be held over until space becomes available (maximum 56 passengers for single deck buses, 14 for the P76 minibus).

Applications must be received by end of office hours each Wednesday in order to start travelling the following Monday week commencing.

Secondary school Priority PhotoCard application form

The forms below are to apply for a Priority PhotoCard for the school year September 2018 to July 2019. Some school bus services require passengers to hold a Priority PhotoCard and some do not.  If you're unsure if one is required, you can check on the individual school pages

Renewals for 2018-19

If you had a Priority PhotoCard for the 2017-18 school year, you can renew this for 2018-19 by completing this online form (opens in a new window). This form must not be used for new starters.

How to appeal against an unsuccessful decision

If you have been unsuccessful in your application or if you feel you have been allocated an incorrect service, you can submit an appeal online.
