Secondary school application forms

The information and application forms on this page are for secondary school services only. (For primary school applications, please see this page.)

Please note that Education Transport staff are following Government guidance are now home working due to Coronavirus. Regretably this means we are unable to post out paper forms by request until further notice.

Application form for school year September 2020 to July 2021 only

This application form is for the current school year ended July 2021 only. Please do not use this form for the new school year starting September 2021 - applications received on the form below will not be accepted. A new online form for 2021/2022 will be published in due course.

To make an application please follow these instructions very carefully. You are advised to use either a desktop or laptop PC to work on the forms - success may vary with tablets and mobile telephone devices. Please read this page for help with completing the forms and and common technical problems.

  • Click one of the links below for the form
  • Save the file to your computer
  • Open the file in your word processor application
  • Complete the form by typing only in the yellow shaded boxes - do not type outside of these boxes
  • Save the file in any format (preferably PDF if possible) and attach to an email
  • Attach to the same email a passport style photograph (any standard image format such as jpeg)
  • Send the email to educationcustomerservices@westyorks-ca.gov.uk with the subject line 'Secondary school application 2020-2021'
  • Both the form and photograph must be sent as attachments to the email and NOT pasted into the text of the body of the email- otherwise we cannot extract the details.
Please ensure you attach both the application form and photograph to the email. These must be as attachments and NOT written into the text of the email. If neither of these steps are done, we cannot process your application.
