Secondary school application form
The application form on this page is to apply for a Priority PhotoCard for secondary school services for the current school year September 2023 - July 2024 and next school year September 2024 to July 2025

This form can be used for both new applications and renewals. However, please do not use this form to apply for or renew statutory zero fare as this requires a different application process (see below). To make an application select the link below. This form is completed online so there are no paper documents to print out.
Before you complete the form, please note the following:
- Please ensure you select the correct academic school year (the first question on the form).
- If your school and/or route is not listed on the form, this may mean an application is not required. Check if your route requires a priority PhotoCard using the A to Z school finder pages.
- Most questions are mandatory and marked with a red *. Ensure you have completed all relevant questions as the form will not submit if any mandatory field has been missed out. An email address and uploading a passport style photograph are both mandatory.
- Please carefully double-check all information is correct before you finally submit the form as mistakes are difficult to rectify once the form has been submitted.
- If you do make an error whilst completing the form before you have submitted it, go back using your browser's back button and correct the mistake.
- After submitting your application, you will receive a confirmation email (sender is "applications@365flexiroute.net"). If you believe you have not received this, please check your "spam" or "junk email" folders. Do not reply to this address. If you have used a work email address, corporate email systems might block the confirmation email entirely. Contact your own IT administrators for advice if this is the case.
