Applying for a Disabled Person's or Blind Person's Pass

Please contact your local Social Services Office for an application form. The main offices in the region are shown below.

Telephone the office in your area to see whether you qualify for a Blind or Disabled Pass. The office will tell you what proofs of identity you will need to supply.

If your application is approved by Social Services, they will complete the order form for the Pass on your behalf, and you will need to provide two recent full-faced, passport-sized colour photographs.

Your new Blind Pass or Disabled Pass will not be issued immediately by Social Services, Travel Centres or Post Offices. Please allow three weeks for delivery to your home address, from the approval date of your application.

Renewals - to renew an out-of-date pass you will need to re-apply through Social Services, who will want to confirm your entitlement.
