You should contact your local Social Services Office for an application form. The main offices in the region are shown below. Telephone the office in your area for advice on rules of eligibility, how to apply, what proofs of identity you need to supply. You will need to provide two recent full-faced, passport-sized colour photographs.
If your application is approved by Social Services for either a bearer only or with/without companion pass, they will complete the order form for the pass, on your behalf. Please allow 3 weeks from approval of your application for it to be delivered to your home address. Your new pass will not be issued immediately by Social Services, Travel Centres or Post Offices.
Lost or damaged cards
If you lose or damage your pass, please contact MetroLine on 0113 245 7676. We will send you out a replacement pass within 7 days but there may be a charge for any lost or damaged cards.
